The Age Friendly Structural Measure was added to the Centers for Medicare & Medicaid Services (CMS) Final Rule on August 8, 2024, and will start with the calendar year (CY) 2025 Reporting Period, affecting fiscal year (FY) 2027 payments. Domain 3 focuses on Frailty Screening & Intervention, requiring hospitals to screen for cognitive impairment/delirium, physical function/mobility, and malnutrition for early detection and intervention. Guided by the Institute for Healthcare Improvement (IHI) Age Friendly 4Ms (What Matters, Medication, Mentation, Mobility), this measure addresses the interconnectedness of these elements. Frail patients are at high risk for geriatric syndromes like falls, delirium, and poor outcomes, making a holistic approach essential for optimal care.
Target Audience: Health care leaders, quality improvement leaders and staff, risk managers, case management, healthcare professionals giving direct patient care, patient safety personnel, physician leaders, and nursing educators.
Speaker: Dr. Bill Bryant is the Chief Quality and Patient Safety Officer at Owensboro Health. Prior to this role, Dr. Bryant practiced Family Medicine in Owensboro for over 30 years. He is certified in Family Medicine and Geriatrics, a Certified Professional in Patient Safety, and a Certified Professional in Healthcare Quality. Dr. Bryant has a special interest regarding patient safety for the frail older patient at risk for delirium.
